FOR CLIENTS HAVING 1:1 Private Sessions

The purpose of processing client data For any clients having personal 1:1 healing/therapy/coaching in order to provide you with the best possible service, I will need to ask and keep personal information about you. I will only use this for my own practice. I will NOT share your information with anyone else without explaining why is it necessary and getting your explicit consent.

The information I hold is your name, email address, phone number, details of any health concerns. During our session your medical history and personal life history discussed and we take notes.

How long I will keep your data for

I will keep your information for a minimum of 7 years according to the recommendation of our insurance company as per below

The records shall be kept for at least a minimum of 7 years following the last occasion on which a treatment was given. - Record Keeping Condition 14c

How we store your personal information

Your information is securely stored on a 3rd party provider, Any personal notes I take about our sessions are stored on my computer which is password protected and only contain your initials so that you cannot be identified. Your personal information (name. Email address & initial health questionnaire) is also stored on Movement Formula Inc platform. As stated above we keep your personal information for a minimum of 7 years following the last occasion of your session. We will then dispose your information by deleting from our files and shredding any paper work.

Therapist’s Rights

Please note that if you don’t agree to me keeping records of information about you, or if you don’t allow me to use the information in the way I need for our sessions, then I may not be able to treat you. I have to keep your records for a certain period as explained, which may mean that if you ask me to erase any details about you, I may not be able to and have to keep them until the required period has passed
